Registered Nurse- Pediatrics Visit Nurse- Home Care

About the role

You will be responsible for the assessment, management, and treatment of assigned patients, visiting clients in the privacy of their residence within designated geographic territories.

Responsibilities

• Schedule and perform monthly supervisory visits.
• Schedule and perform quarterly evaluations.
• Complete timely and accurate clinical notes, including addressing patient's progress.
• Evaluate client status and the skills of workers while the staff member is present.
• Document all communication with clients, family members, and support staff.
• Provide patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
• Counsel and educate the patient and family in meeting healthcare and related needs.
• Write 485/Plan of Care for patients when reauthorizations are due.
• Other projects/duties as assigned.

Requirements

• Home care experience preferred.
• Excellent verbal and written communication skills.
• Highly motivated with organizational, leadership, and relationship-building skills.
• Ability to work in a very detailed, fast-paced environment.
• Currently holds an RN license in PA.
• Valid state driver’s license and car insurance.
