About the role
We are seeking Registered Nurses to join our Team! You will be responsible for the assessment, management, and treatment of assigned patients.
Responsibilities
- Visit clients in the privacy of their residence in designated geographic territories.
- Schedule and perform monthly supervisory visits.
- Schedule and perform Quarterly Evaluations.
- Complete timely and accurate clinical notes including addressing patient's progress.
- Evaluate the client status and the skills of workers while the staff member is present.
- Keep documentation of all communication with clients, family members and support staff.
- Provide patient, family, and/or caregiver education and information pertinent to diagnosis and safety issues.
- Counsel and educate the patient and family in meeting healthcare and related needs.
- Write 485/Plan of Care for patients when reauthorizations are due.
- Other projects/duties as assigned.
Requirements
- Home Care experience preferred.
- Excellent verbal and written communication skills.
- Highly motivated, organizational, leadership and relationship building skills.
- Ability to work in a very detailed, fast paced environment.
- Currently holds a RN license in PA.
- Valid state driver's license and car insurance.
