Role Overview
Provides patient assessment, collects health data, and coordinates individualized care plans to ensure safe and effective interventions.
Responsibilities
- Assesses patients and gathers comprehensive health information involving patients, families, and healthcare teams.
- Develops and coordinates care plans aligned with best practices and patient needs.
- Communicates effectively with patients, families, and colleagues in person, over the phone, and in writing.
- Provides tailored patient and family education based on assessments.
- Participates in quality improvement initiatives to evaluate nursing practice effectiveness.
- Maintains current knowledge of nursing standards, laws, and regulations.
Qualifications & Skills
- Associate's degree or BSN from an accredited institution.
- Current nursing licensure in Illinois.
- CPR certification from the American Heart Association.
