Registered Nurse Navigator Triage - Population Health

About the role

Provides nursing triage services and health advice via phone and in person, assessing patient acuity and support needs. Ensures continuity of care through accurate documentation, patient education, and protocol-driven clinical advice. Coordinates emergency treatments, schedules appointments, and facilitates communication across departments and external organizations. Assists with staff orientation, manages patient concerns, and reinforces treatment instructions to promote patient satisfaction. Maintains timely documentation and suggests process improvements.

Qualifications

• BSN preferred
• 5+ years nursing experience strongly preferred, especially in ICU, ED, or Critical Care
• Specialty certification desired

Skills

• Proficient in computer use, Microsoft Office, SharePoint, Windows, and Teams
• Excellent communication, assessment, coordination, and patient education skills
• Ability to respond effectively to urgent situations and work collaboratively in a team environment

Responsibilities

• Perform patient assessments via phone, documenting symptoms and history
• Coordinate emergency treatments and prioritize urgent calls
• Provide patient education and facilitate scheduling
• Support care coordination across departments and external providers
• Train new personnel and ensure quality communication and documentation
• Manage patient concerns, reinforce treatment plans, and ensure patient understanding
• Suggest improvements to workflows and assist with external care coordination
