About the role
The RN Nurse Navigator Triage provides clinical nursing triage services both over the phone and in person, assessing patient acuity, offering health advice, and coordinating care to ensure quality and continuity. The role is primarily conducted in a clinical environment, emphasizing collaboration, patient-centered care, and adherence to organizational policies.
Responsibilities
- Perform patient assessments and prioritize urgent calls
- Provide health education and support patient understanding of treatment plans
- Schedule appointments and coordinate care across departments and external organizations
- Document patient interactions and manage concerns per established protocols
- Facilitate communication across departments and external organizations
- Train new staff and suggest process improvements
Qualifications
- Graduate of a specialty nursing program
- BSN preferred
- Five years of nursing experience, especially in ICU or ED settings
Skills
- Proficiency in computer skills and Microsoft Office
Schedule
Adaptability to varied shifts required, including evenings and weekends.
