Registered Nurse Navigator Triage - Population Health

Role Overview: The RN Nurse Navigator Triage provides clinical triage services both over the phone and in person, assessing patient acuity, offering health advice, and ensuring continuity of care through education and diagnostic result communication. Key Responsibilities: Perform patient assessments, prioritize urgent calls, coordinate care across departments, provide patient education, facilitate emergency treatments, and document interactions accurately. Support patient satisfaction by managing concerns, explaining medical conditions, and assisting with external organization coordination. Qualifications & Skills: BSN preferred with at least 5 years of nursing experience, ICU/ED background preferred, and specialty certification advantageous. Proficiency in computer systems, Microsoft Office, and telehealth tools is required. Work Environment & Schedule: Work is primarily in a clinical setting, with shifts between 7am and 11:30pm, including every other weekend and holiday rotations.
