Registered Nurse Navigator Triage - Population Health

Role Overview: The RN Nurse Navigator Triage provides clinical nursing triage services, offering health advice via phone and in person, assessing patient acuity and coordinating care. Key Responsibilities: Conduct patient assessments, document symptoms and history, initiate emergency treatments, prioritize urgent calls, educate patients and families, facilitate appointments, coordinate care across departments, support new staff, manage communication with providers and patients, reinforce treatment instructions, address patient concerns, ensure timely documentation, and collaborate with external organizations. Qualifications & Skills: BSN preferred; 5+ years nursing experience, ICU/ED background, specialty certifications, proficient computer skills including Microsoft Office and Teams. Work Environment: Clinical setting with adherence to policies and protocols. Education: Graduate from Specialty Training Program in Nursing; Bachelor's Degree preferred.
