Responsibilities
- Performs comprehensive psychiatric nursing assessments to determine mental status, risk level, clinical symptoms, withdrawal needs, and behavioral functioning.
- Synthesizes psychiatric, medical, and psychosocial data to inform clinical decision-making and plan of care.
- Maintains compliance with observation levels, including timely 15-minute checks and accurate documentation.
- Evaluates medication effectiveness and monitors for side effects or adverse reactions, initiating appropriate follow-up.
- Provides patient education regarding medications and treatment adherence.
- Escalates changes in patient condition and collaborates with providers to adjust treatment plans.
- Oversees patient safety through observation monitoring, rounding compliance, and milieu management.
- Directs and participates in the safe application, monitoring, and documentation of restraint or seclusion interventions when clinically necessary, ensuring compliance with regulatory standards.
- Identifies and mitigates environmental risks and responds promptly to safety concerns or behavioral escalations.
- Ensures adherence to hospital policies, CMS regulations, DNV standards, and state requirements.
- Collaborates with psychiatrists, nurse practitioners, therapists, case management, and nursing leadership to ensure coordinated, patient-centered care.
- Participates actively in interdisciplinary treatment team meetings and contributes to care planning and discharge planning.
- Maintains therapeutic communication with patients, families, and care teams while upholding HIPAA and professional boundaries.
- Demonstrates accountability for clinical practice, decision-making, and patient outcomes.
- Participates in staff huddles, quality initiatives, and continuing education to support clinical excellence.
- Serves as a role model for professionalism, teamwork, and adherence to standards of care.
- Supports unit operations, including admissions, discharges, and patient flow.
Requirements
- BASIC LIFE SUPPORT (BLS)
- REGISTERED NURSE (RN)
- Associate's Degree
- 2+ years related experience
- Arizona state or compact state licensure as an RN
- BLS (AHA)
- Bachelor of Science in Nursing (BSN) preferred
- Experience in behavioral health, psychiatric, or acute inpatient setting
- Crisis Prevention/Intervention (CPI) or equivalent certification
