About the role

Join a Care Coordination team as an RN Hospital Case Manager, providing on-site support to inpatient care teams in a hospital setting. Your role involves assessing, planning, coordinating, and evaluating patient care to ensure optimal health outcomes.

Responsibilities

• Develop individualized care plans.
• Collaborate with patients, families, and healthcare providers.
• Monitor clinical, quality, and utilization metrics to improve care efficiency.
• Assess healthcare, educational, and psychosocial needs of patients or members.
• Design and implement patient-centered care plans with a team approach.
• Monitor progress, laboratory results, and diagnostic tests to evaluate care quality.
• Manage utilization and benefits, serving as a resource to patients and healthcare teams.
• Maintain documentation and adjust treatment plans based on data analysis.

Requirements

• At least two years of RN experience, preferably in a hospital environment.
• Case management experience preferred.
• Ability to work in a clinical environment adhering to organizational policies and procedures.
• Nursing degree from a specialty training program.
• On-site availability.
