Job Duties
- Manage utilization and practice metrics to refine the delivery of care model.
- Full-time, 40 hours weekly, rotating weekends and holidays.
- At least two (2) years of fully licensed RN work experience required.

Position Details
- Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in developing treatment plans.
- Maintains required documentation for all case management activities.
- Collects and utilizes data to adjust the treatment plan when necessary.
Education
- Graduate from Specialty Training Program-Nursing (Required)
- Bachelor's Degree-Nursing (Preferred)
Experience
- Minimum of 2 years-related work experience (Required)
Certification(s) and License(s)
- Licensed Registered Nurse (Pennsylvania) - RN_State of Pennsylvania
