Job Duties
- A typical workweek is Monday through Friday, days. You'll occasionally cover weekends as part of a shared rotation—approximately every fourth weekend.
- At least two (2) years of prior RN experience is required.
- The ideal candidate will have prior RN experience in a hospital setting. Case Management experience is preferred but not required.
- The successful candidate must be available to work on-site at Geisinger Medical Center (GMC).

Position Details
- Affords the opportunity to assess, plan, implement, coordinate, monitor and evaluate all options and services with the goal of optimizing the patient or member's health status.
- Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in the development of treatment plans that are patient-centric, promoting quality and efficiency in the delivery of healthcare for the identified population.
- Maintains required documentation for all case management activities.
- Collects required data and utilizes this data to adjust the treatment plan when indicated.
Education
- Graduate from Specialty Training Program-Nursing (Required)
- Bachelor's Degree-Nursing (Preferred)
Experience
- Minimum of 2 years-Related work experience (Required)
