Registered Nurse - Case Manager

This in-person position involves performing home visits, case management, patient and family education, and creating a supportive environment that respects their wishes. Responsibilities

• Providing expert nursing care and education to patients and families
• Building strong, supportive relationships with patients and the interdisciplinary team
• Managing care plans tailored to individual needs
• Ensuring comfort and quality of life for patients in a home setting

Qualifications

• Vermont RN licensure required
• Minimum 2 years' experience in home health, hospice, or med-surg settings
• Strong communication, assessment, and clinical skills
• Ability to work independently and collaboratively
