Registered Nurse-Hospital at Home (Part-time)

Essential Duties & Responsibilities

  • Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
  • Navigating patients and families across the continuum of care
  • Making audio/visual patient care visits as directed by the care team and/or protocol
  • Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient centered approach
  • Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
  • Adhering to the clinical protocols and being willing to obtain the necessary training to provide care within the context of the providing evidence-based care
  • Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts

Clinical Roles And Responsibilities

  • Providing protocol driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post-operative care
  • Maintaining and practicing in accordance with nursing guidelines per state and hospital regulations
  • Serving as a role model for professional nursing practice
  • Sharing expertise with the multidisciplinary team and serving as a resource to staff in area(s) of expertise
  • Serving as a resource in quality improvement processes and techniques

Quality And Safety Responsibilities

  • Ensuring compliance with reporting requirements for Sentinel Events and Serious Incidents
  • Participating in quality and safety investigations and root cause analyses
  • Taking appropriate action to ensure patient safety as a result of any investigation
  • Serving as a resource in quality improvement processes and techniques

  • Maintains strict adherence to the Lahey Hospital and Medical Center Confidentiality Policy
  • Incorporates Lahey Hospital and Medical Center Guiding Principles, Mission Statement and Goals into daily activities
  • Complies with all Lahey Hospital and Medical Center Policies
  • Complies with behavioral expectations of the department and Lahey

  • Participates in departmental and/or interdepartmental quality improvement activities
  • Participates in and successfully completes Mandatory Education

Education

  • Nursing Diploma or ASN, BSN preferred

Licensure, Certification, Registration

  • A current Massachusetts RN license
  • BLS certification
