Registered Nurse - Hospital at Home (Full-time)

About the role

Reporting directly to the Hospital at Home Director of Nursing, The Hospital at Home Staff RN is responsible for providing high-quality patient care to various patient populations within the framework of the BILH Hospital at Home program.

Responsibilities

• Utilizing monitoring, assessment, planning, intervention and critical thinking skills to care for acute care patient populations in the home
• Navigating patients and families across the continuum of care
• Making audio/visual patient care visits as directed by the care team and/or protocol
• Ensuring all interventions in the patient’s care plan are implemented efficiently with a patient-centered approach
• Leveraging the resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines and documentation systems to support safe, reliable, high-quality, evidence-based care with clinical protocols as the foundation
• Adhering to the clinical protocols and being willing to obtain the necessary training to provide care within the context of providing evidence-based care
• Promoting professional practice and a culture of safety; willingness to engage in process improvement efforts
• Providing protocol-driven assessment, care coordination and interventions as appropriate informed by Hospital at Home protocols for diverse patient populations with a focus on oncology, medicine and post-operative care
• Exhibiting sound clinical decision-making and critical thinking skills
• Maintaining and practicing in accordance with nursing guidelines per state and hospital regulations
• Serving as a role model for professional nursing practice
• Facilitating admissions to the Hospital at Home utilizing Hospital at Home guidelines
• Supporting research efforts to advance knowledge and promote evidence-based practice
• Performing other duties and responsibilities as assigned by the Senior Director of Nursing Quality and Safety

Quality and Safety Responsibilities

• Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents
• Participate in quality and safety investigations and root cause analyses
• Take appropriate action to ensure patient safety as a result of any investigation
• Serve as a resource in quality improvement processes and techniques

Education

• Minimum Qualifications: Nursing Diploma or ASN, BSN preferred

Experience

• Three (3) years RN experience

Work Schedule

• Hours of work may be subject to change based upon the needs of the business/department
• No Shift Rotation
• Weekends

Pay Range

$37.00 - $95.74
