Schedule

  • Every third holiday (Sunday through Saturday)
  • Every third weekend
  • Scheduled Hours: 7:00 PM – 7:00 AM (Night Shift, 12 hours)
  • Hours per week: 24

This role is responsible for planning, coordinating, delivering, and managing direct patient care using the nursing process while adhering to standards of nursing practice to deliver optimal family-centered care. The position also manages and directs day-to-day work responsibilities for the Hospital at Home program to ensure optimal patient flow and satisfaction.

Responsibilities

  • Utilizes the Nursing Process for the delivery and management of patient care from admission to discharge, appropriate to the developmental age of the patient population.
  • Performs patient assessment/reassessment according to standard procedure.
  • Develops and coordinates an individual plan of care, including discharge planning.
  • Develops, implements, coordinates, and documents an education plan for patient/family/significant others.
  • Directs, supervises, coordinates, and evaluates nursing care within the assigned clinical setting.
  • Delegates appropriately to others in accordance with job description and competency level.
  • Complies with established departmental policies, procedures, and objectives.
  • Attends meetings, conferences, and seminars as required or directed.
  • Participates in performance improvement initiatives and demonstrates the use of Quality Improvement in daily operations.
  • Ensures compliance with regulatory agencies such as Joint Commission and DPH.
  • Develops and maintains procedures necessary to meet regulatory requirements.
  • Complies with all health and safety regulations.
  • Respects diverse views and approaches, demonstrates Standards of Respect, and contributes to a professional, tolerant, and civil environment.
  • Develops and maintains established departmental policies, procedures, and objectives.
  • Ensures departmental compliance with hospital-established policies, quality assurance programs, safety, and infection control policies.
  • Maintains open lines of communication with program leadership, other departments, staff, physicians, resident staff, and support personnel.
  • Maintains regular, reliable, and predictable attendance.
  • Performs other similar and related duties as required or directed.

Requirements

  • Current Massachusetts Nursing licensure and registration.
  • Current unrestricted Driver’s license.
  • BLS certification (unless an advanced certification such as ACLS, PALS, or NRP is current). Unit-specific certifications and competencies are defined in the Unit Profile.
  • Minimum of 5 years of nursing experience.
  • Thorough knowledge of the discipline of Nursing.

Preferred Qualifications

  • Bachelor of Science degree in Nursing.
  • Homecare or Telehealth experience.
  • Phlebotomy, IV insertion, and telemetry experience.

Physical Demands and Environmental Conditions

Work is considered heavy. Position requires travel from operations center to local hospitals and patient residences, with primary work being indoors in an alternative patient care environment.

This position may have a signing bonus available; eligibility will be confirmed during the interview process.
