Registered Nurse - Hospital at Home (24 hours/week)

Reporting directly to the Hospital at Home Director of Nursing, the Hospital at Home Staff RN provides high-quality patient care within the BILH Hospital at Home program, delivering inpatient-level care in patients' homes. This is a fully on-site position with field assignments within a 25-mile radius of any BILH hospital, including some virtual nursing from a centralized command center in Charlestown.

Responsibilities

• Utilize monitoring, assessment, planning, intervention, and critical thinking skills to care for acute care patient populations in the home.
• Navigate patients and families across the continuum of care.
• Monitor and respond to biometric data per Hospital at Home Command Center protocols.
• Conduct audio/visual patient care visits as directed by the care team and/or protocol.
• Ensure all interventions in the patient’s care plan are implemented efficiently with a patient-centered approach.
• Leverage resources and expertise of supplier partners to implement collaborative workflows, standards, policies, protocols, guidelines, and documentation systems supporting safe, reliable, high-quality, evidence-based care.
• Adhere to clinical protocols and obtain necessary training to provide evidence-based care.
• Promote professional practice and a culture of safety; engage in process improvement efforts.

Clinical Roles and Responsibilities

• Provide protocol-driven assessment, care coordination, and interventions informed by Hospital at Home protocols for diverse patient populations, focusing on oncology, medicine, and post-operative care.
• Exhibit sound clinical decision-making and critical thinking skills.
• Maintain and practice in accordance with nursing guidelines per state and hospital regulations.
• Facilitate admissions to Hospital at Home utilizing Hospital at Home guidelines.
• Serve as a role model for professional nursing practice.
• Utilize nursing knowledge to identify, prevent, and/or solve complex acute and/or recurring patient care problems.
• Share expertise with the multidisciplinary team and serve as a resource to staff in area(s) of expertise.
• Support research efforts to advance knowledge and promote evidence-based practice.
• Perform other duties and responsibilities as assigned by the Senior Director of Nursing.

Quality and Safety Responsibilities

• Ensure compliance with reporting requirements for Sentinel Events and Serious Incidents.
• Participate in quality and safety investigations and root cause analyses.
• Take appropriate action to ensure patient safety as a result of any investigation.
• Serve as a resource in quality improvement processes and techniques.

Requirements

Education: Nursing Diploma or ASN; BSN preferred.

Licensure, Certification, Registration:

• A current Massachusetts RN license.
• BLS certification.

Experience

Three (3) years of RN experience.

Pay

Pay Range: $37.00 - $95.74 per hour. The pay range listed is the base hourly wage range the organization reasonably and in good faith expects to pay for this position. Actual compensation is determined based on several factors, including seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable.
