This program helps patients avoid preventable ER visits and hospitalizations through advanced, easy-to-use remote monitoring technology and 24/7 access to our Virtual Care Center RNs.
Our home care RNs use cutting-edge technology and clinical knowledge to provide one-to-one care and monitoring in patients' homes. Responsibilities include client assessment, care planning, intervention, and overall supervision of patient care and outcomes. Nurses follow physician orders and comply with the state's Nurse Practice Act, licensure/certification requirements, and our policies and procedures.
Responsibilities
- Provide one-to-one care with patients in their homes
- Conduct client assessments, care planning, and interventions
- Supervise patient care and outcomes
- Follow physician orders and comply with state and organizational regulations
- Utilize advanced technology for remote monitoring and virtual care
Qualifications
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Georgia
- One (1) year of experience as a professional acute care nurse
- Home care experience preferred but not required
