About the role
Provide one-to-one, compassionate care to patients in their homes. St. Mary’s Home Care and Hospice, located in Athens, GA, delivers exceptional home-based care as the area's most comprehensive provider. Our pioneering care model integrates Home Care Connect, a virtual care program that helps patients avoid preventable ER visits and hospitalizations through advanced remote monitoring technology and 24/7 access to Virtual Care Center RNs.
Home care RNs use cutting-edge technology and clinical expertise to assess patients, plan and deliver care, and supervise outcomes in compliance with physician orders, the state’s Nurse Practice Act, and organizational policies.
Responsibilities
- Provide one-to-one patient care in the home setting
- Conduct client assessments and develop care plans
- Implement nursing interventions and monitor patient outcomes
- Follow physician orders and ensure compliance with state and organizational requirements
- Utilize advanced technology for remote patient monitoring and virtual care
Qualifications
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Georgia
- One (1) year of experience as a professional acute care nurse
- Home care experience preferred but not required
