Registered Nurse, Home Health

BAYADA at BAYhealth is seeking a Registered Nurse for full-time or part-time home health visits, assessments, and admissions with adult patients. This office serves clients in Dover, Camden, and Magnolia.

About the Role

As a home care Registered Nurse, you will be an integral member of a multi-disciplinary health care team providing rehabilitative care and skilled nursing to clients, allowing them to receive medical care at home.

Responsibilities

• Follow a designated care plan in accordance with patient's needs.
• Make home visits to clients in designated geographic territories.
• Perform assigned duties, including administration of medication, wound care, treatments, and procedures.
• Monitor clients' conditions; report changes to Clinical or Client Services Manager.
• Follow up with, execute, and properly document doctors' orders.
• Perform client assessments as necessary.
• Case management and coordination.
• Accurately document observations, interventions, and evaluations pertaining to client care using a state-of-the-art touchpad tablet.

Requirements

• A current license as a Registered Nurse in Delaware.
• A minimum of one year of recent, verifiable clinical (medical/surgical) experience.
• Prior home care experience strongly preferred, but not required.
• Graduation from an accredited and approved nursing program, as indicated by school transcript or diploma.
• Ability to work independently and manage time effectively.
• Strong interpersonal skills.
• Solid computer skills; prior experience with electronic medical records (EMR) preferred.
• Ability to travel to cases as assigned.

Pay

Base pay ranges from $62.00 to $67.00 per point, with an annual earning potential of $96,720 to $104,520, depending on qualifications and status.
