About the role
Join a compassionate home health team as a Full-Time Registered Nurse (RN).
Responsibilities
- Manage individualized patient care plans to promote safety and independence at home.
- Provide skilled nursing services such as medication administration, wound care, and patient education.
- Conduct assessments, monitor patient progress, and adjust care plans accordingly.
- Act as the primary liaison between patients, families, physicians, and the care team.
- Coordinate care with interdisciplinary team members and document clinical activities accurately.
- Educate patients and families on health management and safety procedures.
- Communicate patient status updates to ensure timely interventions.
Requirements
- Active RN license in Michigan.
- Over one year of high acuity nursing experience.
- Compassionate, flexible, patient-centered approach.
- Strong organizational, communication, and clinical decision-making skills.
- Valid driver’s license and current CPR certification.
Preferred Qualifications
Experience in home health, hospice, ICU, geriatrics, Med-Surg, ER, telemetry, or related settings.
