Who We're Seeking
We are looking for a goal-oriented RN to join our dedicated Home Health team. The ideal candidate will deliver high-caliber care directly to individuals within the comfort of their homes.
Responsibilities
- Deliver high-caliber care directly to individuals within the comfort of their homes in a designated geographic territory, while also overseeing case management to ensure comprehensive fulfillment of clients' needs.
- Foster collaborative partnerships with colleagues, local physicians, and community providers.
- Establish goals of care and call on the full range of clinical and communication skills to help your clients reach their goals.

Requirements
- Vermont Licensed RN with a passion for providing home health care.
- Prior home health experience and/or case management background preferred, though comprehensive training and orientation will be provided.
- Valid VT driver’s license and reliable vehicle with automobile insurance is essential for home visits.
- Ability to manage time and work independently.
