Role Overview

The registered nurse provides 1:1 home health care to underserved communities, supporting patient recovery and well-being.

Responsibilities

• Assess patient preferences, barriers, and needs
• Develop, document, and communicate individualized care plans
• Collaborate with the interprofessional team and caregivers
• Evaluate patient progress and update care plans as needed
• Provide culturally competent, compassionate care to diverse populations

Qualifications & Skills

• Valid RN license and driver's license
• Associate's degree in nursing (Bachelor's preferred)
• Less than 2 years of experience required; 2-5 years preferred
• Ability to work independently, demonstrate critical thinking, and work in a safety-sensitive environment
