Registered Nurse - Home Health
About the role
As a Home Health Registered Nurse at the Sioux Falls Veterans Affairs Health Care System, you will bring high-quality, Veteran-centered care directly into the home and community setting. This meaningful role allows you to build trusted relationships with Veterans and their families while using your clinical judgment, care coordination, advocacy, and collaboration skills to help Veterans remain safe, supported, and connected to the care they need.
Responsibilities
- Deliver competent, evidence-based patient care to assigned patients and oversee LVNs/NAs as appropriate to the setting.
- Lead in delivering and improving holistic care through collaborative strategies with others and adhere to established standards, policies, and procedures consistent with the interdisciplinary plan of care.
- Utilize proper precautions and assess and chart observations of the Veteran's condition at each visit.
- Complete evaluation tasks including medication review, vital signs, wound care, and other duties within the Registered Nurse scope of practice.
- Coordinate the plan of care with providers and other interdisciplinary team members, focusing on the Veteran in the context of family/community.
- Support decisions, clarify options, and problem solve with the interdisciplinary team, provide consultation and guidance, and act as a liaison for staff, nursing homes, and other health care providers regarding Veteran needs in the community/home setting and interpret Department of Veterans Affairs rules and regulations.
Requirements
- Basic English language proficiency.
- Graduate of a school of professional nursing approved by an accrediting agency and accredited by ACEN or CCNE.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
- Completion of coursework equivalent to a bachelor's degree in Nursing for individuals attending a master's level bridge program.
- Grandfathering provision for those currently employed in VHA in 0610 series and performing duties as described in the qualification standard on 1/29/2024.
Qualifications
- Associate Degree (ADN) or Diploma in Nursing, with no additional professional nursing required for Nurse I - Level I.
- Bachelor of Science in Nursing (BSN), with no additional professional nursing experience required for Nurse I - Level II.
- Master's degree in nursing (MSN) or Master's degree in a related field with a BSN, with no additional professional nursing experience for Nurse I - Level III.
- Doctoral degree in Nursing or Doctoral degree in a related field with a BSN, with no additional professional nursing experience for Nurse II and Nurse III.
