What You’ll Do
- Promote the mission, vision, and values of Weston County Health Services (WCHS)
- Deliver skilled nursing care in the home, including assessment, planning, implementation, and evaluation of individualized care plans
- Maintain accurate, complete patient records, documentation, and required reports
- Administer medications as ordered and monitor for side effects or adverse reactions
- Monitor, report, and document patient symptoms and changes in condition; take and record vital signs
- Coincide care with providers, family members, and other departments to ensure efficient, seamless services
- Provide patient and family education, guidance, and support, including instructions for care and safety at home
- Order/coordinate diagnostic and clinical tests as required and communicate results appropriately
- Support patient comfort and daily needs as appropriate, including mobility assistance and hygiene support when needed
- Adhere to accreditation, compliance, confidentiality, safety, and infection control standards (Standard and Isolation Precautions)
- Participate in staff meetings, in-services, and ongoing professional development
- Maintain a professional, organized work environment; report hazards and replenish supplies as needed
What We’re Looking For
- Current Wyoming Registered Nurse (RN) license in good standing
- Graduate of an accredited nursing program (Associates or Bachelor's degree)
- Current BLS certification (or ability to obtain upon hire)
- Strong clinical assessment and care-planning skills; able to recognize and respond to changes in patient condition
- Knowledge of medical terminology, medication administration, and safe patient care practices
- Excellent documentation and attention to detail; ability to maintain accurate patient records
- Strong communication and interpersonal skills; able to educate patients and collaborate with families and providers
- Able to work independently, manage time effectively, and prioritize multiple responsibilities
- Professional judgment, integrity, and commitment to confidentiality and compliance standards
- Able to handle challenging situations calmly and professionally
- Able to meet physical requirements of the role, including occasional lifting/carrying up to 50 lbs.
- Flexibility for occasional after-hours calls and schedule variations as needed
