What You’ll Do

  • Promote the mission, vision, and values of Weston County Health Services (WCHS)
  • Deliver skilled nursing care in the home, including assessment, planning, implementation, and evaluation of individualized care plans
  • Maintain accurate, complete patient records, documentation, and required reports
  • Administer medications as ordered and monitor for side effects or adverse reactions
  • Monitor, report, and document patient symptoms and changes in condition; take and record vital signs
  • Coincide care with providers, family members, and other departments to ensure efficient, seamless services
  • Provide patient and family education, guidance, and support, including instructions for care and safety at home
  • Order/coordinate diagnostic and clinical tests as required and communicate results appropriately
  • Support patient comfort and daily needs as appropriate, including mobility assistance and hygiene support when needed
  • Adhere to accreditation, compliance, confidentiality, safety, and infection control standards (Standard and Isolation Precautions)
  • Participate in staff meetings, in-services, and ongoing professional development
  • Maintain a professional, organized work environment; report hazards and replenish supplies as needed

What We’re Looking For

  • Current Wyoming Registered Nurse (RN) license in good standing
  • Graduate of an accredited nursing program (Associates or Bachelor's degree)
  • Current BLS certification (or ability to obtain upon hire)
  • Strong clinical assessment and care-planning skills; able to recognize and respond to changes in patient condition
  • Knowledge of medical terminology, medication administration, and safe patient care practices
  • Excellent documentation and attention to detail; ability to maintain accurate patient records
  • Strong communication and interpersonal skills; able to educate patients and collaborate with families and providers
  • Able to work independently, manage time effectively, and prioritize multiple responsibilities
  • Professional judgment, integrity, and commitment to confidentiality and compliance standards
  • Able to handle challenging situations calmly and professionally
  • Able to meet physical requirements of the role, including occasional lifting/carrying up to 50 lbs.
  • Flexibility for occasional after-hours calls and schedule variations as needed
