Registered Nurse - Home Health

Position Overview

Sought is an experienced Registered Nurse (RN) to deliver in-home nursing services in Moline, IL and surrounding areas. Duties include direct patient care, documentation, and independent care management.

Key Responsibilities

• Provide direct skilled nursing care to patients in their homes
• Perform comprehensive patient assessments and ongoing evaluations
• Complete accurate and timely documentation using HomeCare HomeBase
• Deliver care in accordance with individualized care plans and physician orders
• Maintain monitoring of patient conditions and communicate changes to appropriate team members
• Education of patients and families on disease processes, medications, and care plans
• Ensure compliance with agency policies and regulatory standards
• Participate in on-call rotation and respond to patient needs as required

Required Qualifications

• Active Registered Nurse (RN) license
• Home health experience
• Experience with HomeCare HomeBase (HCHB)
• Strong clinical assessment and documentation skills
• Ability to work independently and manage a caseload

Preferred Qualifications

• OASIS experience and/or certification
• Strong organizational and time management skills
• Excellent communication and interpersonal abilities
• Dependable and self-motivated work style
• Ability to adapt to a fast-paced, field-based environment

Pay & Schedule

• Pay Rate: $51.00 to $53.00 per hour
• Weekly Hour Guarantee: 32 hours
• On-call/Weekend Requirement: Yes, participation in on-call rotation required
