Responsibilities
- Work in the Cardiology Outpatient Practice with primary focus in the Heart Failure Clinic; also provide coverage for Amyloid and Sarcoid Clinics.
- Engage in face‑to‑face and non‑face‑to‑face interactions with patients.
- Be accountable for nursing care, including pre‑visit preparation, direct patient care, patient/family education, and transitions of care.
- Communicate and collaborate with providers and other Care Team members.
- Support professional nursing practice across practice settings and across the continuum of care to meet patient and family needs.
- Function within the Mayo Nursing Care Model, which includes accountability for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients
