Registered Nurse – Heart Failure Clinic

Role Overview: The Care Team RN provides comprehensive nursing care within the Cardiology Outpatient Practice, primarily in the Heart Failure Clinic, along with coverage for Amyloid and Sarcoid Clinics. Key Responsibilities Perform face-to-face and non-face-to-face patient interactions, including pre-visit preparation, direct care, and patient/family education.Coordinate transitions of care and collaborate effectively with providers and team members.Support professional nursing practice across various settings, adhering to the Mayo Nursing Care Model and ANA standards.Lead and mentor through roles such as preceptor, participate in quality improvement, and delegate care based on skill, experience, and patient needs. Qualifications & Skills Graduate of an accredited nursing program with at least one year of RN experience, preferably with cardiac care experience.Excellent verbal and written communication, teamwork, and critical thinking skills.Proficiency in computer use and electronic medical records preferred.Current RN licensure, BLS certification, and ability to work flexible hours are required.
