About the role

The Community Health Nurse provides comprehensive, high-quality nursing care to assigned patients, ensuring safety, continuity, and patient well-being.

Responsibilities

• Conducting ongoing assessments of physical, emotional, social, cultural, and environmental needs
• Developing and updating personalized care plans
• Coordinating home health services
• Monitoring patient conditions and implementing physician orders
• Educating patients and families on self-care, medications, and treatments
• Supervising support staff
• Ensuring documentation accuracy
• Maintaining compliance with healthcare regulations and safety protocols
• Participating in quality improvement activities, ongoing education, and agency meetings
• Upholding confidentiality and patient rights

Qualifications

• BSN from an accredited program
• At least one year of acute care RN experience
• Valid New York State nursing license
• Strong clinical skills
• Adherence to safety and privacy standards
• Ability to perform independently in a home healthcare setting
