About the role
The Community Health Nurse is responsible for delivering comprehensive, high-quality nursing care to assigned patients in their homes, ensuring safety, continuity, and patient satisfaction.
Responsibilities
- Provide professional nursing care using all elements of the nursing process in line with standards and policies.
- Assess patients’ physical, emotional, social, cultural, economic, and environmental needs.
- Monitor patient conditions, report changes, and plan individualized care and discharge strategies.
- Supervise supportive staff and delegate tasks appropriately.
- Implement physician orders for medications and treatments, and instruct patients and families in self-care techniques.
- Maintain accurate documentation of care provided and monitor patient satisfaction.
- Participate in quality improvement activities, agency meetings, and ongoing professional development.
- Ensure confidentiality, compliance with HIPAA, and adherence to safety protocols.
Qualifications
- BSN from an accredited nursing program.
- At least one year of RN experience in an acute care setting.
- Current RN license and registration in the state.
