About the role

The Community Health Nurse is responsible for delivering comprehensive, high-quality nursing care to assigned patients in their homes, ensuring safety, continuity, and patient satisfaction.

Responsibilities

  • Provide professional nursing care using all elements of the nursing process in line with standards and policies.
  • Assess patients’ physical, emotional, social, cultural, economic, and environmental needs.
  • Monitor patient conditions, report changes, and plan individualized care and discharge strategies.
  • Supervise supportive staff and delegate tasks appropriately.
  • Implement physician orders for medications and treatments, and instruct patients and families in self-care techniques.
  • Maintain accurate documentation of care provided and monitor patient satisfaction.
  • Participate in quality improvement activities, agency meetings, and ongoing professional development.
  • Ensure confidentiality, compliance with HIPAA, and adherence to safety protocols.

Qualifications

  • BSN from an accredited nursing program.
  • At least one year of RN experience in an acute care setting.
  • Current RN license and registration in the state.
