Registered Nurse - Full-Time

Role: Community-Based Home Health RN responsible for providing comprehensive, patient-centered care in patients' homes. Key Responsibilities: Manage multiple patients with varying acuity levels, conduct post-hospitalization assessments, facilitate admissions and discharges, provide patient education, perform skilled nursing tasks such as dressing changes, IV therapy, and access site care. Ensure safety and support with services like lifeline and meals on wheels, and document all care accurately using the Epic system. Position Details: Deliver high-quality care with autonomy and meaningful patient impact. Care for diverse patient populations across different backgrounds. Offers flexible scheduling, mileage reimbursement, and on-call responsibilities. Qualifications: Minimum one year of related experience preferred. Must have an accredited RN license in Missouri, BLS certification through the American Heart Association, a valid driver’s license, and proof of auto insurance. Skills & Attributes: Compassionate, adaptable, strong communication, and documentation skills. Ability to work independently while maintaining high standards of care.
