Registered Nurse Coordinator - Primary Care

About the role

This position involves serving as an expanded nurse within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management. The nurse provides primary clinical support to patients, families, and providers, assisting with diagnosis, treatment, and patient interaction management.

Responsibilities

• Assess healthcare, educational, and psychosocial needs; develop personalized care plans.
• Facilitate communication among patients, providers, and healthcare team to ensure effective treatment.
• Educate patients and families on disease management, care coordination, and health promotion.
• Implement clinical interventions based on risk stratification and evidence-based guidelines.
• Collaborate with providers to enhance adherence to clinical guidelines, initiate therapies, and monitor outcomes.
• Coordinate diagnostic tests, maintain documentation in electronic health records, and manage patient data.
• Participate in professional development, review current literature, and incorporate best practices.
• Assist with laboratory specimen collection, medication administration, IV management, and patient education.

Qualifications

• Graduate from a specialty training program in nursing; Bachelor's Degree preferred.
• At least two years of RN experience, preferably in Med/Surg or hospital settings.
• Strong communication, clinical judgment, and teamwork skills.
