Registered Nurse Coordinator - Primary Care - Part Time

Role Overview: This position involves serving as an expanded nursing role within an interdisciplinary team to promote preventative care, conduct screenings, coordinate disease management, and support primary clinical functions. The nurse assesses patient needs, educates patients and families, facilitates communication among healthcare providers, and manages clinical interventions based on evidence-based guidelines. Additionally, the role includes coordinating diagnostics, maintaining documentation, and participating in professional development. Responsibilities Assess healthcare, educational, and psychosocial needs of patients and families.Develop and implement individualized treatment plans, involving patients in goal setting.Coordinate communication among care team members and investigate healthcare options.Educate patients and families on disease management, care coordination, and health strategies.Assist with clinical interventions, including medication administration, IV management, and diagnostic testing.Document activities accurately in electronic health records and maintain compliance with protocols.Participate in professional development and quality improvement initiatives.Support post-discharge transitions and care coordination efforts. Qualifications & Skills Minimum of 2 years RN experience; Med/Surg hospital experience preferred.Graduate from a Specialty Training Program in Nursing; Bachelor's degree preferred.Strong clinical, communication, and patient education skills.Ability to collaborate effectively with healthcare teams and manage multiple responsibilities.
