Registered Nurse Coordinator - Primary Care

Position Overview: This role involves serving as an expanded nursing provider within an interdisciplinary team to promote preventative care, conduct screenings, and coordinate disease management. The nurse will support primary care providers, assist with patient triage, and develop individualized care plans based on patient needs. Key Responsibilities Assess healthcare, educational, and psychosocial needs of patients and families, involving them in treatment planning.Facilitate communication among patients, providers, and healthcare team to eliminate barriers and identify interventions.Educate patients and families on disease management and care coordination using age-appropriate methods.Implement clinical interventions following evidence-based guidelines and risk stratification.Collaborate with providers to enhance guideline adherence, initiate therapies, and monitor patient progress.Coordinate diagnostic tests, maintain documentation in electronic health records, and collect patient data.Participate in professional development, review current literature, and contribute to care coordination program initiatives.Assist with procedures such as IV insertion, medication administration, and specimen collection within scope of practice. Qualifications & Skills Graduate from a Specialty Training Program in Nursing; Bachelor's Degree preferred.Minimum of two years RN experience, preferably in Med/Surg or hospital settings.Strong communication, clinical, and patient education skills.Ability to work collaboratively in a team environment and adapt to diverse patient needs.
