Registered Nurse Coordinator - Primary Care - Part Time

About the role

This position involves serving as an expanded nurse within an interdisciplinary team to promote preventive care, conduct screenings, coordinate disease management, and support primary clinical functions. The nurse assesses patient needs, develops individualized care plans, educates patients and families, and facilitates communication among healthcare providers to ensure effective treatment.

Responsibilities

• Performing clinical interventions, managing laboratory and diagnostic tests, and maintaining documentation in electronic health records
• Coordinating patient care activities, including medication administration, IV management, and specimen collection
• Supporting disease management programs through data collection, patient education, and adherence to clinical guidelines
• Participating in professional development, evidence-based practice, and care transition coordination
• Assisting in designing and evaluating care programs aligned with organizational goals

Qualifications

• Graduate from a Specialty Training Program in Nursing (required)
• Bachelor's Degree in Nursing (preferred)
• Minimum of two years of RN experience, preferably in Med/Surg settings
• Strong clinical, communication, and patient education skills
• Ability to collaborate effectively with healthcare teams and utilize evidence-based practices
