Registered Nurse Case Manager: Home Health

Responsibilities
- Create, implement, and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
- Assess patients and families upon initial visit, develop advanced plans of care, and teach patients and family members about wound care, blood draws, tube feedings, tracheostomy care, foley catheters, congestive heart failure management, and medication education.
- Collaborate with interdisciplinary clinical teams to manage patient care episodes.
- Prepare clinical notes, orders, and charting in a timely manner.
- Initiate and administer IV medication (antibiotics and diuretics) safely and effectively according to established protocols.
Requirements
- Current and unrestricted California RN license.
- Current BLS/CPR certification.
- Valid California Driver’s License.
- High level of clinical knowledge in home health care.
- Experience with high acuity patients.
- Experience working with an interdisciplinary team.
- Case Management fundamentals.
- Previous experience with IV Medication infusions.
- Proficiencies initiating intravenous (IV) therapy.

Pay
Registered Nurse, Level 1: $130,000 - $160,000 annually, plus a 10K annual bonus, paid quarterly.
Schedule
Full-time position with flexible work schedules within a 3-10 mile radius of Yuba City and surrounding cities.
