Registered Nurse Case Manager: Home Health

About the role

A dynamic Registered Nurse Case Manager position is available in Santa Rosa, California. This role will provide comprehensive care coordination for patients receiving home health services.

Responsibilities

• Assess patient needs and develop care plans
• Coordinate with interdisciplinary team to ensure seamless patient care
• Manage patient transitions between care settings
• Provide education and support to patients and families
• Document care activities and progress in electronic health records

Requirements

• Bachelor's degree in Nursing (BSN)
• Current California RN license
• Minimum 2 years of acute care nursing experience
• Experience in home health or related field preferred

Qualifications

• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Proficient in Microsoft Office Suite
