About the role

Responsibilities

  • Create, implement, and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
  • Assist and teach patients and their family/caregivers with various medical procedures including wound care, blood draws, tube feedings, tracheostomy care, foley catheter management, and medication education.
  • Perform a full assessment of the patient and family upon initial visit, using assessment data to develop an advanced plan of care including rehabilitation.
  • Work with patients and family one-on-one to achieve improvement and/or stability, case managing their care episodes with an interdisciplinary clinical team.
  • Prepare clinical notes, orders, and charting in a compliant time frame, collaborating with physicians and agency staff to deliver the best care possible.
  • Initiate and administer IV medication (antibiotics and diuretics) safely and effectively in accordance with established protocols.

Requirements

  • Current and unrestricted CA RN license.
  • Current BLS/CPR Certification.
  • Valid CA Driver's License.
  • High level of clinical knowledge in home health care.
  • Experience with high acuity patients.
  • Experience working with an interdisciplinary team.
  • Case Management fundamentals.
  • Previous experience with IV Medication infusions.
  • Proficiencies initiating intravenous (IV) therapy.

Pay

Registered Nurse, Level 1: $130,000 - $160,000 annually, plus a 10K annual bonus, paid quarterly. Starting pay is commensurate with relevant experience above the minimum requirements.

Schedule

Full-time position with flexible work schedules and autonomy.
