About the role
Provide clinical case management services to patients in their homes.
Responsibilities
- Assess patient needs and develop care plans
- Coordinate with healthcare providers to ensure continuity of care
- Monitor patient progress and adjust care plans as necessary
- Communicate with patients and families to provide education and support
- Ensure compliance with regulatory standards and insurance requirements
Requirements
- Bachelor's degree in Nursing (BSN)
- Current California RN license
- At least 1 year of home health nursing experience
- Strong communication and interpersonal skills
- Proficiency in Microsoft Office Suite
Qualifications
- Experience working with Medicare/Medicaid patients preferred
- Knowledge of home health agency policies and procedures
- Ability to work flexible hours including evenings and weekends
