Registered Nurse Case Manager, Case Management, Per Diem, Shift Varies

About the role

The Pool RN Case Manager position supports the physician and interdisciplinary team in facilitating patient care, aiming to enhance clinical outcomes and patient satisfaction while managing the cost of care.

Responsibilities

• Facilitate the collaborative management of patient care across the continuum, intervening as necessary to remove barriers to timely and efficient care delivery and reimbursement.
• Apply process improvement methodologies in evaluating outcomes of care.
• Coordinate communication with the healthcare team, patients, families, and post-acute care services.
• Readmission screening and prevention.

Qualifications

• Registered Nurse (RN)
• 3 years of hospital clinical experience with 1-3 years of hospital or payor Case management experience preferred.
• Excellent interpersonal communication and negotiation skills, strong analytical, data management, and computer skills.
• Current working knowledge of discharge planning, utilization management, case management, and performance improvement.
• Understanding of pre-acute and post-acute venues of care and post-community resources.
• Strong organizational and time management skills, as evidenced by capacity to prioritize multiple tasks and role components.
• Ability to work independently and exercise sound judgment in interactions with the health care team and patients/families.

Pay

$47.00 / hour, estimated, depending on shift as applicable.

Schedule

Shifts may vary and may include: 8:00 a.m. – 4:30 p.m., 9:00 a.m. – 5:30 p.m., 8:00 a.m. – 6:30 p.m., 8:00 a.m. – 8:30 p.m.
