Registered Nurse Case Manager

About the Role

The Home Care Registered Nurse Coordinator PACE plans, organizes, and implements in-home services for elderly participants. This role acts as a liaison between home health agencies and the PACE program, ensuring high-quality, efficient care through assessment, coordination, and collaboration.

Responsibilities

• Coordinate day-to-day in-home services for elderly participants.
• Assess participants in their homes and develop individualized care plans.
• Provide treatments and health education to participants.
• Utilize telehealth and community resources to support care delivery.
• Collaborate with an interdisciplinary team to ensure compliance and maintain documentation.
• Act as a liaison between home health agencies and the PACE program.

Requirements

• Bachelor of Science in Nursing (BSN); those with an Associate Degree or Diploma must obtain a BSN within 6 years.
• Current RN licensure with the North Carolina Board of Nursing.
• Valid driver’s license and reliable transportation.
• Minimum of 2 years' experience in interdisciplinary settings.
• At least 1 year of experience working with elderly or frail populations.

Preferred Qualifications

• Experience with Electronic Medical Records (EMR).
• Experience with telehealth platforms.
• Community home-based nursing experience.
• Mental health experience.
