Registered Nurse Case Manager

About the role

As a Registered Nurse Case Manager at Dove Healthcare you will work with employees, patients, family members, and other professionals to coordinate all aspects of providing person‑centered care. You will mentor, train, and educate a team of caregivers while overseeing the care of assigned residents.

Responsibilities

• Supervise the day‑to‑day activities of assigned residents and employees.
• Coordinate, plan, implement, and evaluate nursing services for each resident using clinical competence.
• Review and audit compliance with each patient’s plan of care and changing needs.
• Provide a safe, comfortable, and home‑like environment.
• Participate in an on‑call rotation.
• Coordinate all aspects of delivering person‑centered care.

Requirements

• Current licensure in the State of Wisconsin as a Registered Nurse (RN).
• Degree from an accredited school of nursing.
• Previous nursing experience.

Schedule

Full‑time position; limited‑term with potential to become permanent; includes participation in an on‑call rotation.

Pay
