Responsibilities

  • Participates in multi-disciplinary rounds to review patient status, progress, and care level, and discharge plans.
  • Educates patients and families on the emotional, social, and financial impacts of illness, and mobilizes family/community resources to meet identified needs.
  • Affirms patient and family empowerment in making health care decisions and accessing needed services.
  • Assesses readmitted patients for their perceived reasons for readmission and organizes and facilitates patient and family care conferences with the multidisciplinary team.
  • Documents discharge planning evaluations, ongoing assessments, discharge plans, MDRs, barriers to care, avoidable days, and patient and family needs according to standard work.
  • Communicates with payors to secure authorization for post-acute care as needed.

Requirements

  • Associate's degree in Nursing [Required]
  • Bachelor's degree in Nursing [Preferred]
  • 2+ years of medical/hospital nursing experience [Required]
  • Prior experience in Care Management/Utilization Management [Preferred]
  • Registered Nurse (RN) [Required]
  • Certified Case Manager (CCM) [Preferred]
  • Accredited Case Manager (ACM) [Preferred]

Schedule

Full-time, Monday-Friday, 8a-4:30p, Rotating weekends and holidays.

Pay

$35.19 - $62.17
