About the role
Provides comprehensive care for patients with chronic or acute heart failure in an outpatient clinic setting.
Responsibilities
- Assess patients through physical exams, symptom review, and vital sign monitoring.
- Educate patients and families on heart failure management, medication adherence, and lifestyle modifications.
- Coordinate care plans with healthcare providers, including medication titration per protocols.
- Manage incoming calls to evaluate symptoms, modify care, or refer to physicians.
- Act as a liaison between physicians and patients, facilitating follow-ups and diagnostic tests.
Qualifications
- Licensed Registered Nurse credentialed from the applicable state nursing board or eligible for licensure.
- BLS certification obtained within 30 days of hire, accepted from AHA or ARC.
- Diploma from an accredited nursing school or equivalent professional licensure at hiring.
