About the role

Registered Nurse in the Advanced Heart Failure Clinic, responsible for assessing and managing patients with heart failure, providing education on disease management, coordinating care plans with providers, and acting as a liaison between physicians and patients.

Responsibilities

• Perform physical exams, monitor vital signs, and evaluate symptoms.
• Educate patients and families on medication adherence, lifestyle modifications, and disease management.
• Coordinate care, including medication titration and follow‑up appointments.
• Manage incoming calls to assess symptoms, adjust care, or refer to physicians.
• Facilitate communication between healthcare providers and patients for diagnostic tests and follow‑up.

Qualifications

• Licensed Registered Nurse credential from the Tennessee Board of Nursing or eligible.
