Responsibilities

  • Establish, organize, direct, and implement the Cardiac Rehabilitation Program in accordance with Federal, State, and local guidelines.
  • Evaluate and treat patients with a wide variety of cardiac disabilities in an outpatient setting, helping to achieve risk factor reduction in cardiovascular disease using therapeutic exercises, activities, and educational classes.
  • Document all patient care appropriately in the medical record to comply with department policies and JCAHO requirements.
  • Orient and perform initial assessment of participants in the cardiac rehabilitation program.
  • Develop plans of care for new participants.
  • Document initial assessment and daily participant information and progress.
  • Lead and/or delegate participant education sessions based on staff expertise and identified participant needs.
  • Develop and maintain current educational materials for patient education.
  • Assess participants pre-, during, and post-exercise.
  • Supervise exercise sessions and/or delegate based on staff experience.
  • Obtain monthly required approval of individualized treatment plans from program medical director, per Medicare guidelines.
  • Monitor participants for inappropriate or abnormal responses to exercise.
  • Make appropriate referrals and/or recommendations to participants, the participant’s personal physician, and/or program Medical Director as needed or as requested by patients.
  • Report any significant participant conditions (dysrhythmias, abnormal blood pressure response, change in participant’s status, etc.) immediately to the physician supervising the program.
  • Initiate ACLS measures and/or emergency care according to facility policy.
  • Check AED equipment and other emergency supplies and equipment for proper function and adequate supply per facility policy.
  • Set up and supervise emergency drills and mock codes on a routine basis, giving appropriate feedback and constructive criticism where appropriate.
  • Optimize reimbursement for cardiac rehabilitation services through effective communication with all cardiac rehabilitation staff, including support and/or office staff.
  • Coordinate and/or maintain Continuous Quality Improvement program and participate in or delegate participation in quality reporting program per facility policy.
  • Maintain current knowledge in the field of cardiac rehabilitation through resources including but not limited to: cardiac rehabilitation literature, in-services, continuing education courses and/or conferences.
  • Coordinate any routinely occurring meetings, including organizing and planning guest speakers, education, food, and/or public communications.

Education

  • Must be a graduate of an accredited school of nursing.
  • All RNs hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor’s prepared nursing program within 4 years of hire.
  • BSN must be completed within 10 years.
  • RNs licensed on or before 12/19/17 are/remain exempt from this requirement.

Experience

  • Minimum 1-year relevant cardiac experience, preferred.

Licensure/Certifications

  • Current NYS License as a Registered Nurse.
  • Current BLS certification.
  • Current ACLS certification within 6 months from date of hire.

Physical Requirements

  • Medium Work - Exerting up to 50 lbs. of force occasionally, and/or up to 20 lbs. of force frequently, and/or up to 10 lbs. of force constantly to move objects.
  • Occasional: balancing, climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, repetitive motion, stooping, visual acuity (color, depth, perception).
  • Frequent: typing/clerical/dexterity, hearing, standing, talking, walking.
  • Constant: seeing/monitor/computer use, fit testing.

Hazards

  • Occasional bodily fluids/bloodborne exposure.

Working Conditions

  • Occasional extreme temperatures.

Travel

  • No travel required.

Bonus

  • RN (excluding management) sign-on bonuses are awarded in the amount of $20,000 for full time.
  • LPN sign-on bonuses are awarded in the amount of $10,000 for full time.
  • Eligible Employers: A.O. Fox Hospital, Bassett Medical Center, Cobleskill Regional Hospital, Little Falls Hospital, O'Connor Hospital, Valley Health Services and Valley Residential Services.

Pay

  • Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards.
  • Pay Range: $37.49/Hr. – $56.24/Hr.
