Registered Nurse - Assisted Living

About the Role

The registered nurse (RN) coordinates nursing care, including direct patient care, patient and family education, and managing transitions of care. The RN supports professional nursing practice across various settings and throughout the continuum of care to meet the needs of patients and their families. Operating within a professional practice model, the RN is accountable for planning, implementing, evaluating, and communicating all phases of nursing care for assigned patients.

The RN provides leadership through roles such as preceptor, informal leadership, and quality improvement initiatives. They delegate patient care based on skill level, experience, patient acuity, fiscal responsibility, and resource availability. Excellent communication skills are essential, as well as the ability to mentor and teach. The RN may also participate in committees or projects.

Responsibilities

• Coordinate and deliver direct patient care.
• Provide patient and family education.
• Manage transitions of care.
• Plan, implement, evaluate, and communicate nursing care for assigned patients.
• Delegate patient care based on skill level, experience, acuity, and resource availability.
• Serve as a preceptor, mentor, and informal leader.
• Participate in quality improvement initiatives.
• Engage in committees or projects as needed.

Requirements

• Experience in Assisted Living and Short Term Rehab required.
• Long-term care experience preferred.

Schedule

• Shift options: 6AM - 6PM or 6PM - 6AM.
• Every 3rd weekend required.
• 13-week assignment.
