Registered Nurse Assessment Coordinator (MDS) (RNAC)

LVN wage range: $33–$38 hourly. Salary range for RN: $79,040–$91,520 per year.

About the role

The Registered Nurse Assessment Coordinator (RNAC, MDS) coordinates patient/resident care from admission through discharge under the direction of the Director of Nursing.

Responsibilities

• Identify and monitor resident skilled services
• Coordinate the Resident Assessment Instrument (RAI) process
• Complete accurate Minimum Data Set (MDS) and develop the interdisciplinary Plan of Care
• Monitor patient/resident status to identify changes in condition, barriers to discharge, and progress toward care goals
• Participate in the daily Medicare Prospective Pay System (PPS) Interdisciplinary Team Meeting (ITM)
• Establish and maintain positive relationships with all members of the healthcare team and function as part of the nursing management team

Requirements

• Current, unencumbered RN (or LVN) license in state of practice
• Minimum of 2 years of direct hands-on nursing experience
• Minimum of 2 years of long-term care experience
• Preferably at least 1 year of RAI experience (focus in MDS)
• Proficiency in ICD-10 coding
• Reliable transportation
