Key Responsibilities

• Utilize the nutrition care process to conduct a comprehensive assessment for residents upon admission and routine reassessment.
• Facilitate nutrition related care for residents on both short-term rehab and long-term care units.
• Collaborate with interdisciplinary care team to determine appropriate plan of care for residents.
• Monitor resident's dietary intake, weight, lab values and habits.
• Provide nutrition education and counseling to residents and families.
• Participate in care plan meetings, quality improvement initiatives and nutrition programs.
• Ensure compliance with state, federal and facility regulations.
• Complete MDS assessment in compliance with state guidelines.
• Collaborate with dietary staff for menu planning and to oversee foodservice operations as needed.

Qualifications

• Registered Dietitian (RD) credential required.
• Bachelor's degree in Dietetics, Nutrition or related field
• Current state licensure (or eligibility) in New York
• Previous experience in clinical setting preferred
