About the Role

We are seeking a dedicated and knowledgeable Registered Dietitian (RD/RDN) to join our team. In this role, you will be responsible for assessing residents’ nutritional needs, developing individualized care plans, and collaborating with interdisciplinary teams to ensure optimal patient outcomes.

Responsibilities

• Conduct comprehensive nutritional assessments for residents/patients
• Develop, implement, and monitor individualized nutrition care plans
• Provide medical nutrition therapy in accordance with physician orders
• Collaborate with physicians, nursing staff, and other healthcare professionals
• Ensure compliance with federal, state, and facility regulations
• Monitor and document residents’ nutritional status and progress
• Participate in care plan meetings and interdisciplinary conferences
• Educate residents, families, and staff on nutrition-related topics

Requirements

• Registered Dietitian (RD/RDN) credential required
• Current state licensure (or eligibility) required
• Bachelor’s degree in Dietetics, Nutrition, or related field
• Experience in long-term care, skilled nursing, or clinical settings preferred
• Strong communication and organizational skills
• Ability to work independently and as part of a team
