We are seeking a talented Registered Dietitian to join our multidisciplinary team at the Daniel Driscoll Neponset Health Center, delivering holistic care while achieving key performance measurements in a community setting.

The successful candidate will provide dietetic services for all ages and across diverse socio-economic and cultural groups in an organization committed to inclusion, innovation, and impact. This part-time position offers 20 hours weekly with flexible days and times, including both telehealth and in-person visits. In-person services may be provided at our Boston, Plymouth, or Hyannis health center locations.

A $2,500 sign-on bonus is offered, with half paid at hire and the remainder after 90 days in good standing (former employees of Harbor Health Services are not eligible).

Responsibilities

  • Provide optimal nutrition care to patients by obtaining diet history to determine past and current eating habits and performing assessments of nutrient needs based on diet history, provider referral, and/or medical record review.
  • Develop and implement individualized, evidence-based nutrition care plans, including short- and long-term goals; evaluate effectiveness and plan follow-up care as needed.
  • Collaborate with an interdisciplinary care team (MDs, NPs, RNs, behavioral health, pharmacy, and social workers) to deliver integrated nutrition care focused on clinical outcomes in areas such as diabetes, weight management, and heart disease.
  • Maintain accurate records of nutrition care, document in the medical record per agency policies, and communicate pertinent data to ensure continuity of care with referring providers.
  • Provide medical nutrition therapy, nutrition education, and food access resources to patients in the Ryan White program.
  • Assist in developing and implementing nutrition education and referral programs for the food pantry and other low-income clients, including quarterly nutrition workshops and cooking demonstrations.
  • Select and develop culturally appropriate education materials.

Requirements

  • Bachelor’s Degree in Nutrition.
  • Registered Dietitian (RD) and Licensed Dietitian Nutritionist (LDN) in the state of Massachusetts (required).
  • Certified Diabetes Educator (CDE) preferred.
  • 1 year of experience in a healthcare setting.
  • Ability to communicate in a second language (Spanish, Vietnamese, or Portuguese) preferred.
  • Self-motivated with the ability to work in a collaborative, team-based environment.
  • Excellent written and verbal communication skills.
  • Familiarity with Electronic Medical Records applications (EPIC preferred).
  • Basic skills in Microsoft Word, Excel, and Outlook.
